Paperwork: For your first appointment and to make the most of our time together, paperwork must be completed prior to the appointment start time. 

Arriving on Time: Scheduling an appointment reserves a specific block of time to be spent directly with your care. Please arrive on time. We are unable to extend your appointment time.

Cancellations: Cancellation of an appointment is required by 5pm the business day prior to the scheduled appointment. Cancellations after 5pm or appointments not cancelled/rescheduled may be charged a fee.

Consistency: It is important that there be consistency to treatment once begun. Treatment may be discontinued if there is a history of missed or late canceled appointments.

Payment: Payment is expected at the time of service. All co-payment, deductable or co-insurance amounts are quoted by your insurance company and final determination of a claim will be made by your insurance company after the appointment. The patient is responsible for charges not covered or reimbursed by the insurance company.
